Parents are responsible for informing the school nurse each school year of any medical conditions, such as asthma, diabetes, seizure disorder, or severe allergies, that may affect your student's welfare while at school. For students who suffer from a chronic condition or potentially life-threatening allergy, a parent must sign a Care Plan completed by the student's physician and submit it to the school nurse. If your student requires medication to be administered during the school day, please complete the Medication Administration Form and bring the medication in the original bottle or container with the prescribed information on the label. Medication Administration Forms are also required for any emergency medication such as epipens, glucagon, or inhalers.
